- Your pet's name and date of birth
- Previous veterinarian's name and phone number
- List of medications to transfer
- Your contact information
How It Works:
We'll contact your previous vet to obtain medical records and prescription information. Most transfers complete within 2-3 business days.
Note: Some prescriptions may require an exam with Dr. Sherman before refills are authorized, especially controlled substances or medications requiring regular monitoring.
